Patients Know Best (PKB) replaced the Your EPR patient portal at Calderdale and Huddersfield NHS Foundation Trust last Monday, 30th September. It complements our new DrDoctor appointment management system, giving patients the ability to view and manage their health information and care from CHFT in one place, improving their experience.
PKB brings together patient data from health and social care providers and the patient's own recorded data into one secure health record, empowering patients to better manage their health at home, or wherever they may be.
Patients simply register and log in to the secure online system using any internet-enabled device, such as a smartphone, tablet or desktop computer.
They can then view their healthcare information using both systems together, such as hospital appointment information and appointment letters (through DrDoctor), allergies and some diagnostic test results (through PKB). The latter will be particularly helpful for patients with chronic conditions such as diabetes, as being able to access their own test results from PKB will help with the self-management of conditions.
Patients can also securely access their hospital appointments electronically via the NHS App. The service, which is completely free, links hospital information from Patients Know Best in the NHS App, allowing them to view appointment details.
Anyone who has been a patient at CHFT (aged 16 and over) will be sent an invitation to register for PKB from email address no-reply-uk@patientsknowbest.com.
