What Happens next
Once the team has received a referral the CCNT will make contact with the family to discuss the
referral and what is required.
If appropriate telephone advice and support may be given to the family.
If a visit is required the time frame and location is always negotiated on an individual family basis. At the visit the nurse will determine with the child and family the appropriate plan of care to meet the child’s needs. Some children may only require one episode of care, others have many episodes.
Once the child’s episode of care has been completed the CCNT will inform parents/carers of
discharge and sign post families to other appropriate services if they are required.
